El trauma en el cuerpo

En el cuerpo están escritas todas las experiencias que nos han resultado traumáticas. Un “trauma” es un choque emocional capaz de producir un daño duradero en el inconsciente. Vivencias de esta naturaleza nos afectan como totalidad, y por eso influyen en todos los aspectos de nuestras vidas. Desde la sombra, el trauma dificulta la integración y la coherencia, tanto al interior de nosotros mismos como en las relaciones que establecemos.

El trauma puede derivar de experiencias de maltrato infantil, que ocurre cuando un padre o cualquier otro adulto causa un daño físico o emocional a un niño en el contexto de una relación de responsabilidad, confianza y poder. En esto está incluida la exposición del hijo a situaciones de violencia entre los padres.

El maltrato causa estrés y se asocia a trastornos del desarrollo cerebral temprano. En casos extremos puede verse alterado el desarrollo de los sistemas nervioso e inmune. En consecuencia, los adultos que han sufrido maltrato en la infancia corren mayor riesgo de sufrir discapacidades o enfermedades de diversa índole.

El maltrato ubica al niño como víctima de violencia en cualquiera de sus manifestaciones y puede ser físico, emocional o psicológico, y sexual:

En el maltrato físico se incluyen: golpes, latigazos, ataduras, encadenamiento, quemaduras, mordeduras, colocar al niño bajo la ducha fría o mantenerlo sumergido en el agua, lanzarle un objeto o utilizar un objeto para golpearlo, hacerle pasar hambre o no proporcionarle comida, zarandearlo, encerrarlo.

El maltrato emocional o psicológico tiene un efecto negativo en el desarrollo emocional y en la autoestima. En él se incluye ignorar al niño o no demostrarle sentimientos de amor, apoyo u orientación, amenazarlo, atemorizarlo, denigrarlo o criticarlo constantemente.

Otra forma de maltrato es la negligencia, que consiste en cualquier acción, o inacción, de la persona a cargo del cuidado de un niño, que le cause un daño físico o emocional. En este ítem se incluye cualquier cosa que interfiera con su desarrollo y crecimiento, como no proveer comida, abrigo o una vivienda adecuada, no proporcionar atención médica ante una lesión o enfermedad, encerrarlo en una habitación o un placard, exponerlo a una situación de peligro donde pueda lesionarse físicamente o morir.

El abandono es un tipo de negligencia que ocurre cuando se deja a un niño solo durante un período de tiempo prolongado, o cuando sufre un daño serio porque nadie lo estaba cuidando.

El abuso sexual es cualquier tipo de contacto sexual con un niño, o cualquier comportamiento que tenga como fin la excitación sexual del adulto, utilizando como objeto al niño. No sólo implica mantener relaciones sexuales con él, tocar sus genitales o hacer que el niño le toque los genitales, sino también hacer que pose para fotografías pornográficas o videos, contarle chistes o historias con connotación sexual, mostrarle material pornográfico, obligarlo a desnudarse, «exponerse» ante él o mostrarle los genitales.

En muchos casos el abuso ocurre dentro del hogar, y los autores son las personas encargadas de la crianza y desarrollo, en otros, personas allegadas a la familia o del ámbito educativo o de esparcimiento. Al niño le resulta difícil hablar pues queda atrapado en el doble mensaje de los adultos que, al mismo tiempo, parecen amarlo pero lo maltratan, y en su doble sentimiento de afecto positivo hacia ellos y temor por el poder que ejercen sobre él.

Los adultos que abusan sexualmente de los niños suelen conocerlos previamente y estar relacionados con ellos de alguna manera. Generalmente, utilizan esta relación a su favor ejerciendo sobre la víctima su poder de seducción, pidiéndole que mantenga en secreto lo que comparten, o amenazándolo con que le pasará algo malo si se lo dice a alguien, o le pasará algo malo a alguien a quien él quiere.

Algunos indicadores emocionales de un niño abusado sexualmente, o de los niños que son testigos del abuso (pero que no son las víctimas directas, como por ejemplo hermanos) pueden actuar como alertas para un observador atento:

Estar triste o enojado. Estar retraídos, temerosos, deprimidos, o tener poca autoestima o hacerse daño, como por ejemplo cortarse. Los niños más deprimidos pueden contemplar el suicidio o intentar suicidarse. Algunos niños se dedican a intimidar a otros y tienen problemas para controlar su agresividad y otras emociones. Muchos de ellos tienen pesadillas o problemas para dormir y regresión en el control de los esfínteres (niños que habían superado esa etapa vuelven a ella).

Problemas de relación. Problemas para relacionarse con otros y mantener relaciones. Por lo general falta de confianza en los demás, especialmente cuando se trata de adultos. Una señal preocupante es cuando el niño no busca el consuelo en los padres o personas encargadas de su cuidado.

Mal comportamiento o conductas de riesgo. Es posible que pierdan interés en actividades que antes disfrutaban, no se concentren en la tarea escolar y sus calificaciones empeoren, presenten problemas de disciplina o se pongan en peligro al adoptar conductas de riesgo.

Erotización temprana. Los chicos y chicas expuestos a abuso sexual y experiencias sexuales desviadas, conductas, actitudes y conocimiento sexual inapropiado poseen riesgo de erotización temprana y sexualidad precoz. Pueden, así mismo, experimentar su primera excitación sexual en el momento del abuso.

Hay conductas que tienden a  exacerbar el erotismo en niños, niñas y adolescentes como: estimular modos de comportarse, poses y actitudes  eróticas propios de personas jóvenes o adultas, el consumo de ropa,  juguetes, películas,  personajes de ficción, dibujos animados, cosméticos, juegos, eventos, programas televisivos, redes sociales, publicidad, concursos  y eventos que hacen énfasis en los atributos eróticos en lugar de resaltar las características propias de la niñez y adolescencia.

Algunos signos a observar: masturbación compulsiva, provocación erótica o conducta sexual inapropiada.

La presencia de algunos de los síntomas descriptos en los casos anteriores no evidencia, necesariamente, una situación de abuso, pues los niños que están atravesando situaciones conflictivas, como la separación o divorcio de los padres, una mudanza familiar, o la pérdida de un amigo o miembro de la familia, suelen sufrir cambios de humor o de conducta y presentar algunos de los desequilibrios mencionados. No sucede lo mismo con la erotización temprana que mayoritariamente está indicando alguna forma de abuso sexual.

Ante una sospecha de maltrato o abuso los adultos tienen que hacerse responsables y realizar las consultas correspondientes para confirmar la problemática y determinar un accionar que aleje al niño de esa situación. Cuanto antes se identifique y detenga el maltrato, menos destrucción creará. Es importante tener en cuenta que todas las acusaciones realizadas por un niño merecen atención.

Muchas veces los indicadores son notados en el ámbito educativo o los padres concurren a la consulta de un terapeuta por alguna otra razón y se devela la situación, pero enorme es la cantidad de adultos que concurren a la terapia sin haber vuelto a contactar con esa experiencia y, poco a poco, van abriendo la ventana a ese trauma que tan profundas huellas le ha dejado.

Cuesta imaginar que alguien pueda lastimar intencionalmente a un niño, pero las estadísticas dan cuenta de lo contrario, y eso basándose en los casos que se denuncian, pues la mayoría no llegan a esa instancia, o pasan desapercibidos ya que, por lo general, los niños tienen temor de contar lo que les sucede y pedir ayuda.

Todo niño, para crecer y desarrollarse de manera saludable necesita saber que puede confiar en quienes, en una relación asimétrica, son responsables de su crianza, necesita recibir de sus mayores un modelo basado en valores de vida y conducta coherente.

Según la OMS (Organización Mundial de la Salud), los estudios internacionales revelan que aproximadamente un 20% de las mujeres y un 5 a 10% de los hombres manifiestan haber sufrido abusos sexuales en la infancia, mientras que un 23% de las personas de ambos sexos refieren maltratos físicos cuando eran niños. Además, muchos niños son objeto de maltrato psicológico (también llamado maltrato emocional) y víctimas de desatención.

Las estadísticas de un estudio realizado en Bolivia reflejó que 8 de cada 10 bolivianos/as viven algún tipo de violencia física, sexual y/o emocional dentro del seno familiar (ENDSA, 2008). Recientemente, en el mismo país y durante el proceso de elaboración de la Ley Contra la Trata y el Tráfico de Personas, se evidenció que 3 de cada 10 mujeres y 1 de cada 10 varones viven alguna forma de violencia sexual y que el 70% de los casos denunciados ha sido sufrido por niños y niñas menores de 12 años.

Consecuencias del maltrato

Los que lo sobreviven al maltrato infantil continúan sufriendo emocionalmente aún cuando las lesiones físicas se hayan superado, y suelen presentar signos de padecer un estado crónico de estrés postraumático. Los indicadores son tensión y miedo, aún cuando no haya un peligro visible o este haya pasado, imágenes recurrentes e incontrolables del suceso o el sentimiento de que el evento está sucediendo nuevamente, dificultad para dormir o pesadillas, sentimiento de soledad, explosiones de ira, sentimientos de preocupación, culpa o tristeza.

Los estados postraumáticos son generadores de defensas, polarizadas en la rigidez o en el caos, que solo están al servicio de prolongar la devastación de un trauma del pasado, aún cuando ya no se encuentren presentes los factores que lo provocaron.

Una vivencia traumática puede bloquear el movimiento innato del ser humano hacia la integración y la coherencia. El trauma implica al área de las emociones, y estas afectan al cuerpo y a la mente. Toda emoción no tramitada queda guardada en el cuerpo en forma de sensaciones que, si no son atendidas, devienen en síntomas.

El hecho de poder ponerle palabras a la experiencia ayuda a desbloquear esa memoria traumática, pero esto no siempre es posible pues la herida puede haber tenido lugar en etapas previas al desarrollo del lenguaje, cuando el aparato psíquico no estaba aún conformado; o es posible también que la magnitud de la experiencia haya provocado una amnesia infantil funcional a la dificultad para afrontar en esa etapa lo sucedido.

Lo que el cuerpo guarda, solo el cuerpo puede liberarlo, por esto son recomendables las disciplinas que propician la toma de conciencia de la experiencia sensorial y cinestésica en el aquí y ahora: expresión corporal, sensopercepción, eutonía, gimnasia consciente, método Feldenkrais, bioenergética, entre otras. Otra vía regia de sanación la constituye el arte en todas sus manifestaciones, pues favorece la liberación de esa energía tóxica albergada en el interior de quien ha padecido la lesión emocional.

Una terapia que incluya esta mirada, puede estimular el desarrollo de una conciencia corporal que no interprete ni juzgue y solo observe el fenómeno. En ese proceso, las defensas se debilitan y se facilita el desbloqueo. Elhecho de dirigir la atención al lenguaje de nuestro cuerpo, sus sensaciones y movimientos, nos posibilita el acceso a mayor información sobre nosotros mismos y nuestra biografía.

Un riesgo que esconde el haber padecido un tipo de apego que incluye agresión, maltrato y/o abuso, es el de percibir el maltrato como algo natural, y a la violencia como una forma válida de relacionarse con los demás y resolver problemas. La vivencia de terror, impotencia y falta absoluta de control sobre los acontecimientos seguirá rigiendo el modo de accionar ante la vida hasta que la toma de conciencia, y un trabajo de transformación, ayuden a superarla.
